What is concierge primary care?

Concierge primary care is a more personalized approach to healthcare. My practice is intentionally small so that I can offer longer visits, greater accessibility, and a strong focus on prevention and whole-person care.

What does my membership include?

Your membership provides enhanced access to your primary care clinician, longer and more personalized visits, convenient communication, care coordination, preventive health planning, and other services and amenities described in your membership agreement.

Do you accept insurance?

Yes. Horizon Primary Care is designed to complement your health insurance, not replace it. Where applicable, we will bill insurance for covered medical services. Your membership fee covers services and amenities that are separate from

insurance-covered care.

Can I be a Medicare patient?

Yes, if your circumstances and our practice structure permit. Medicare patients may have specific rules regarding billing and membership fees. We will clearly explain how Medicare billing and your membership work before you enroll.

Will I still need health insurance?

Yes. A concierge membership does not replace health insurance. We recommend maintaining appropriate health insurance coverage for specialists, hospital care, diagnostic testing, medications, and other services outside the practice.

What if I need a specialist or hospital care?

We remain involved in your care. We can help coordinate referrals, communicate with specialists, obtain records, and help you navigate the healthcare system when you need care outside our practice.

How quickly can I reach my clinician?

One of the benefits of a smaller practice is improved access. Members have convenient ways to communicate with the practice and can typically expect a more timely response than is common in traditional primary care.

Do you offer telehealth?

Yes. Telehealth appointments are available when clinically appropriate. In-person visits are also available at our office.
